Just fill in the areas where it says <six digit color code> with a hex code of your choice and replace the header image with the url to your image.


EDIT: In the box you'll recieve a comment like this "<H3>This form is inoperational!</H3> <Provisional users can't create forms name="e297">"

You need to replace that with this: <form name="e297">
in order to make the layout work properly.
