Ummm that's kinda a vague question... To make a layout you must first learn how to code and I suggest you start with basic html at http://www.lissaexplains.com/html.shtml and move on to CSS from there. Using those two languages you an make a super page.
If you can't be bothered to learn you can make a webpage using a program ....they can be limiting and expensive though but look into it if it intrests you. I do however suggest you learn rather than use a program because it's always better, you can do exactly what you want and you can fine tune your layouts.

You start off in a graphics program. The most common one for beginners is paint shop pro (abbreviated psp) and you can download a free trial at jasc.com (http://www.jasc.com). You can find images in many places, the most popular being tonystone.com (http://www.tonystone.com). It is, however, not exactly legal to take their photos to use..so keep that in mind. The best way is to use something you've created graphically or scanned onto your computer. You can mess with the effects to make your layout look unique. A popular quick way of snazzing up graphics is to use brushes, which can be downloaded everywhere, just search the forum or google.
Depending on how your layout is set up, you need to code it using mainly frames and tables, which you can read about here at lissaexplains.com As someone said earlier, you can use a program to do the work for you (i think dreamweaver, one of the programs you mentioned, is used to do this but im not entirely sure) or you can learn yourself.
Do you have any more specific questions?

try using different effects, if you go to http://www.jennyspage.net you'll find a bunch of tutorials on different effects to use in PSP. You can take a picture and use a bunch of effects on the picture to create your layout.
