For the banner part, don't you just do
<img src="urlhere">
near the top of your Custom Header box? And about the image thing, if I'm right about what I think you're thinking, it should be like this:
<img src="yoururlhere" ALT="What you want it to say when you mouseover image">

if you want to make the words on the banner, like on the image with different fonts, you need an image editor like photoshop or paint shop pro or the gimp.
