this is pretty easy to do and I am sure this works. First, make your form, then, where you want the image submit button, put this:
<input type="image" src="imageaddress.gif" onclick="submit()">
You will need to edit the imageaddress.gif with your image address and then it should work.

and heres how to do a clear button:
<input type="image" src="imageaddress.gif" onclick="reset()">
